Gift Aid Declaration

This declaration confirms my wish to make this and all future payments to the Catholic Medical Association (UK), whether by way of subscription for membership or donation, under the Gift Aid Scheme. This applies to all such payments made by me from 6th April 2014.

I understand I must pay an amount of income tax or capital gains tax in the relevant year equal to any tax reclaimed by the Catholic Medical Association in that period.
